Exclusions
This is a discount plan, not a dental insurance plan. Exclusions apply:
- It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit or Cherry.
- This plan is only honored at Beachwood Dental. It cannot be used at any other dental or specialist office.
- This plan cannot be used for hospitalization or hospital charges of any kind.
Member Limitations
- Any unused services within the patient membership are non-transferable to other plan members regardless of the service.
- Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
- All payments are due in full at the time services are rendered.
- No refunds.
Program Guidelines
- No other discounts with this offer.
- Membership is not for resale.
- All membership fees are due at the time of registration and are non-refundable.
- Total family plan membership fee does not exceed a 5 member household.
- The need for Prophylaxis cleaning vs Periodontal scaling and root planing will be assessed at the time of exam.
